Insurance Plans
We accept most insurance plans. We file insurance claims only to your primary insurance. You are advised to check with your insurance company regarding coverage prior to registering with us. You are responsible for knowing the requirements and benefits of your insurance plan. If our doctors refer your child for additional testing or to a specialist, please check with your insurance company before scheduling and going to the appointment or test. We ask you to complete an information sheet to update your insurance information at the first appointment of the new year and at any time your personal or insurance information changes. You are responsible for all office co-payments at the time of check-in (please read our co-pay policy). There is an administrative charge added to co-pays not paid at the time of the visit. We accept checks, cash or credit cards for your convenience. If you have a large deductible or a health savings account we ask that you pay a portion upfront. We can always work out a payment agreement if necessary.
Co-Pay Policy
As a courtesy to our customers, we have agreed to file all claims to your primary insurance provider. We are contractually obligated by the insurance companies to collect all co-pays at the time of service. It is the customer’s responsibility to update our office with any changes to their medical insurance policy. Your co-pay must be paid each time your child is seen in our office as indicated by your insurance. We realize that circumstances may sometimes warrant a delay in payment, therefore, the following policies apply:
- All co-pays not paid at the time of service will be assessed a $5.00 fee.
- Payment of the co-pay must be made as soon as possible after your visit to our office, preferably within 10 business days.
- Co-pays not paid within 30 business days from the date of service will be assessed an additional $5.00 fee.
- Any abuse of the co-pay policy (i.e., habitual non-payment or delayed payment) may result in re-scheduling of appointments.
It is not the primary function of our staff to act as a collections office. Please be prompt in all of your payments. This will allow everyone to focus on what is really important, the health of your child.
